SSA-44 IRMAA Appeal Form: Lower Your Medicare Surcharge

The form moves through five steps. Step one names your event and its date — the date matters, because it anchors which year’s income Social Security will substitute. Step two is the heart of the form: your estimate of this year’s MAGI (adjusted gross income plus tax-exempt interest), with your filing status. Step three lets you add next year’s estimate if your income will fall again — the common pattern when a mid-year retirement leaves a partial salary year followed by a full retirement year — so one filing can correct both.

Steps four and five are evidence and signature. The evidence covers two things: that the event happened — a letter from your employer giving your last day, a divorce decree, a death certificate — and that your estimate is grounded, through pension award letters, your benefit statement, and last year’s 1099s.

Build the estimate from real components rather than a hopeful round number; Social Security later compares it against the return you actually file, and a figure that proves far too low gets adjusted back afterward. The form is also revised periodically, so download the current version free at ssa.gov/forms rather than reusing a saved copy.
