To appeal Medicare IRMAA, file Form SSA-44 — “Medicare Income-Related Monthly Adjustment Amount — Life-Changing Event” — with Social Security, at no charge, as soon as a qualifying event such as retirement reduces your income. You do not need to wait for the determination letter: retire in March, and you can file in March, which prevents months of overcharge rather than recovering them afterward.

The long answer
You can even file SSA-44 during the same visit where you enroll in Medicare, so the surcharge correction and the enrollment paperwork travel together. The filing itself is a few pages: your event and its date, a documented estimate of this year’s income, and evidence such as an employer letter stating your last day. File it at your local office, mail it certified with photocopies kept, or handle it by phone appointment at 1-800-772-1213.
While the appeal is processed — straightforward filings generally resolve within several weeks — keep paying the billed premium; an appeal in progress does not pause the bill, and unpaid premiums put coverage at risk. If the appeal is approved, your premium is corrected going forward and the surcharge already collected that year is refunded or credited. A denial isn’t the end: you have 60 days to ask Social Security to reconsider.
One more expectation to set: every year gets its own determination. If you retire in 2026, your 2027 premiums will be set by your 2025 return — which may still show peak income — so one retirement can take two SSA-44 filings before the surcharge fully clears. That is normal, and not a sign the first appeal failed.
