Your 2026 IRMAA is based on the income from your 2024 tax return.

The main table shows the six 2026 IRMAA tiers for single filers and married couples filing jointly. Each row includes the Part B premium, the Part D surcharge, the monthly total, and the yearly total for that tier.

Married filing separately IRMAA brackets for 2026
The married filing separately table applies only if you filed separately and lived with your spouse at any time during the year.

This filing status has only two IRMAA tiers. If your 2024 income was $109,001 to $390,999, your 2026 Part B premium is $649.20 and your Part D surcharge is $83.30. If your income was $391,000 or more, your Part B premium is $689.90 and your Part D surcharge is $91.00. One dollar over $109,000 jumps straight to $529.60 a month. Head of household filers, qualifying surviving spouses, and married filing separately filers who lived apart all year use the single table instead.
IRMAA brackets are cliffs
IRMAA is a cliff, not a gradual increase. One dollar over a bracket line puts you in the next tier for the whole year. For example, $274,001 of 2024 income for a married couple filing jointly raises the monthly surcharge from $95.70 to $240.40, according to the CMS 2026 Medicare premium tables. The surcharge applies per person on Medicare, so a couple can pay it twice.
What to do if you are over an IRMAA line
If your income has dropped because of a life-changing event, such as retirement, Form SSA-44 asks Social Security to use your current income instead of the income from two years ago. The form is free at ssa.gov/forms.
If your income is simply high, IRMAA is decided again every year. For future years, people usually take planning questions to a tax professional, including which accounts to draw from, when to realize gains, and the size of a Roth conversion. The Part D surcharge is billed by Medicare, not your drug plan, and it applies even if you chose a $0 premium plan.
