What most people call the IRMAA exception form is officially Form SSA-44, “Medicare Income-Related Monthly Adjustment Amount — Life-Changing Event” — Social Security publishes no form actually named “exception.” SSA-44 is the exception route for people whose income dropped after a qualifying life event such as retirement; a separate correction path, with no form at all, covers cases where the tax data Social Security used was wrong or outdated.

The long answer
The SSA-44 route covers eight recognized events: you retired, cut back your work, married, divorced or had a marriage annulled, lost your spouse, had a pension stop or shrink, lost income-producing property through circumstances you didn’t choose, or received an employer settlement payment out of a bankruptcy or reorganization. The form runs a few pages, is free at ssa.gov/forms, and is filed with proof of the event plus support for your estimate of this year’s income.
The second exception needs no life event: if Social Security worked from the wrong numbers — you amended your return, the IRS corrected it, or a newer return exists — call 1-800-772-1213 or visit your local office with the corrected return or IRS transcript and ask for a new determination.
What is not an exception, stated plainly: a one-time windfall, a required minimum distribution, or a market loss. No form changes those — though a single unusual year drops out of the calculation on its own after one cycle, because each year’s premiums are set fresh from a newer return.
