A life-changing event for IRMAA is one of eight situations Social Security accepts as grounds to recalculate the income surcharge on your Medicare premiums: retirement (the rules call it work stoppage), a reduction in work, marriage, divorce or annulment, a spouse’s death, the loss or reduction of a pension, the loss of income-producing property outside your control, and an employer settlement payment. Retirement is the most common by a wide margin — you stopped working, your income fell, and the two-year-old tax return Social Security used still shows your working years.

The long answer
Matching your situation to the list is usually simple. Recently retired: your event is work stoppage, documented with an employer letter stating your last day plus proof of what your income looks like now, such as a pension award letter or your Social Security benefit statement. Recently widowed: the event is the death of your spouse, documented with the death certificate — and since your filing status may change too, the form accounts for both. A divorce works the same way, with the decree as evidence.
The event’s date matters — it anchors which year’s income Social Security will use in place of the old return. You report one event per form, alongside your estimate of this year’s MAGI: adjusted gross income plus tax-exempt interest, built from real documents.
And a plain word about what doesn’t count: selling a home, a large Roth conversion, a required minimum distribution, a market drop. Those aren’t life-changing events under the rules — though a single high-income year clears the formula by itself after one cycle, because every year’s premium gets its own determination from a fresher return.
